Therefore, please send the following information ONLY to info@tcmfit.com for the check. 1) Photos of the front and back of your insurance card 2) Patient’s date of birth. 3) Photos of primary care physician referral letter (if insurance requires)
For initial appointments, please fill 3 forms: 1) informed consent form, 2) new patient registration, and 3) Traditional Chinese Medicine Questionnaire.
For follow-up appointments, please fill 2 forms: 1) informed consent form, and 2) progress form or Medical Necessity Review (MNR) form before you come. Please complete the designated forms to avoid any delay or fees for your treatment.
