We accept many insurances. We need to do an insurance eligibility check before scheduling your initial insurance paid acupuncture appointment. Therefore, please send the following information ONLY to info@tcmfit.com for the check. 1) Photos of the front and back of your insurance card 2) Patient’s date of birth. 3) Photos of primary care physician referral letter (if insurance requires)
For initial appointments, please fill 3 forms: 1) informed consent form, 2) new patient registration, and 3) Traditional Chinese Medicine Questionnaire.
I strongly suggest that you call the number on the back of your insurance card and verify your own benefits. Ask the following questions:
• Does my plan cover the treatment (acupuncture)?
• Do I have: A deductible? Co-pay? How much do I am responsible for?
• Is there a limit to the number of visits or dollar amount for acupuncture?
• Do I need a referral from my Primary Care Physician?
• Does my plan cover treatment for my specific health issue?
