We need to do an insurance eligibility check before scheduling your acupuncture with an insurance initial appointment. Therefore, please send the following information ONLY to info@tcmfit.com for the check. 1. Insurance plan name, 2. Insurance member ID, 3. Patient’s full name, 4. Patient’s date of birth, 5. Primary care physician referral letter (photocopy), 6. photos of the front and back of your insurance card.
For initial appointments, please fill 3 forms: 1) informed consent form, 2) new patient registration, and 3) Traditional Chinese Medicine Questionnaire.
For follow-up appointments, please fill 2 forms: 1) informed consent form, and 2) progress form or Medical Necessity Review (MNR) form before you come. Please complete the designated forms to avoid any delay or fees for your treatment.
